About the Program


The Illinois Premise Alert Program allows people with special needs to provide their information to be kept in a database for Fire, Police and EMS agencies and personnel in the State of Illinois. The information can then be provided to responders dealing with a situation involving a special needs person.

The notification expires 2 years after the date the form was submitted so be sure to renew it every 2 years by filing a new form. You can renew the form as many times as necessary.
